Doctor Michel Rouif operates every day except Thursday, and practices plastic surgery under agreement as well as cosmetic surgery outside the convention at the Pôle Santé Léonard-de-Vinci.

This establishment, opened in 1998 and resulting from the grouping of four Tours clinics, is one of the most important private care centers in the Center-Val de Loire region. Risk prevention in general

The collection of medical and surgical history at the start of the consultation allows a better appreciation of the operative risk.

The quality of this information, which should be exhaustive if possible, is therefore essential.

Prevention of negative effects of smoking

It has been established that smoking increases the risk of postoperative complications by 2 to 4, and in particular that of infection and skin necrosis. This increased risk disappears when smoking is stopped completely during the 6 to 8 weeks preceding the operation, and the 2 to 4 weeks following it. Smoking permanently impairs capillary blood circulation.

The consequences linked to smoking are mainly surgical complications, the deterioration of healing leading to suffering, even skin necrosis, the increase in chronic pulmonary pathologies, the 4-fold increase in the risk of infection at the surgical site.

Prevention of thromboembolic risk (phlebitis, pulmonary embolism)

The constitution of a venous thrombosis is multifactorial. Among the very many risk factors for thromboembolic complications linked to the patient, we should mention: obesity, smoking, the existence of varicose veins, immobility, age and a history of venous thrombosis and pulmonary embolism.

Venous thrombosis (or phlebitis) is the constitution of a blood clot in a vein, most often in the calf. Favored by immobilization and by dehydration, it is often painful. Above all, it carries a risk of embolism, that is to say that the clot is detached from the vein and, carried by the blood stream, goes up to the heart, which then sends it to the lungs. This "pulmonary embolism" can be fatal.

The mechanical means implemented in the context of postoperative prevention involves elastic restraint, i.e. wearing stockings, socks or compression bands without forgetting intermittent pneumatic compression (IPC) which results in an acceleration of venous return by leg or leg and thigh sleeves, inflated by a compressor and then deflated at regular intervals. Intermittent pneumatic compression is started at the start of the operation and is continued postoperatively by wearing an elastic stocking until active mobilization (benefit of getting up early).

If the risk is high enough, drug prevention by subcutaneous injection of anticoagulant (low molecular weight heparin) is carried out during the week following the operation.

Prevention of surgical site infections

Surgical site infections may be limited:

• By avoiding shaving of the operating site in the 15 days preceding the operation. Prefer chemical hair removal or clippers and only at the incised site.
• By good personal hygiene. For this reason, a preoperative shower is prescribed with an antiseptic foaming soap the day before, but also the day of the intervention.
• By the surgeon's experience
• By draining the operating site, controlling diabetes, stopping smoking.

A single injection of antibiotic at the start of the procedure (antibiotic prophylaxis) has proven its effectiveness for many procedures. Prevention of postoperative nausea and vomiting (PONV)

Without prevention, patients present for 50 to 90% of postoperative nausea and vomiting. They consider them to be more uncomfortable than pain, decreasing the satisfaction of the intervention.
Four elements are considered to be risk factors: female, non-smoker, with a history of PONV, subject sensitive to motion sickness.

Prevention of high blood pressure

The occurrence of arterial hypertension outbreaks during certain procedures can increase the risk of hematoma and therefore bleeding (lifting). This is why it is important to do the preoperative check-up with the anesthesiologist seen in consultation and, if necessary, the cardiologist (blood pressure, electrocardiogram, biological check-up, or even more if necessary).

Prevention of hypothermia
